The guides above explain different kinds of support and the questions you may want to ask:
Our Support at Home service guide explains the broader program, including clinical care and other service categories.
An aged care assessment looks at your needs and the support you are eligible for. If you are approved for Support at Home, your decision letter and support plan explain the services and funding approved for you. Our access guide walks you through the process. You can also read the My Aged Care program information.
Let your provider know which days and times matter to you. If you go to a regular community activity, you may need help getting ready before you leave as well as support during the outing. If you have a family carer, include their commitments too. Allow time for travel and rest, and agree on a schedule that fits your approved support and budget.
Ask for a written plan showing the visits, their cost and any contribution you will pay. The full service price and your own contribution are different amounts. Our funding guide explains how they work, and our pricing guide helps you compare quotes. Keep a copy of the agreed prices and schedule so you can refer to them later.
You may also want to learn about equipment and home modifications, the Restorative Care Pathway or the End-of-Life Pathway. These have their own assessment and funding arrangements. Your assessment or care team can help you explore whether they suit your needs.
Tell us where you live, the help you are looking for and your preferred days and times. You are welcome to involve someone you trust. We can discuss your enquiry and confirm which services are available in your area before you make arrangements.
