Tell your assessor which everyday tasks are becoming difficult. You might want to move around your home more easily, use the bathroom with greater confidence or prepare a meal. Explain what gets in the way and what help you already have. This gives your assessor a starting point for finding equipment or changes that suit you.
An aged care assessment identifies your AT-HM needs. Funding for approved equipment and home modifications is separate from your ongoing Support at Home budget. Some equipment needs a prescription from a qualified health professional, and home modifications require an occupational therapist’s prescription. The official AT-HM guide explains the process.
Choice provides this page as an information guide. Arrange AT-HM assessments, equipment and home modifications with an appropriate AT-HM provider. Before buying an item or booking work, ask that provider to confirm approval, costs and payment arrangements in writing.
You should feel comfortable using the equipment in the places where you need it. Ask your provider or health professional whether a trial would help you decide. Talk through these questions together:
If you are considering a bathroom change, tell the occupational therapist what is difficult about your usual routine. They can assess the space and recommend an appropriate modification. The position and type of a rail, for example, need to suit you and your home.
Before work starts, agree on the work to be done, any property permissions, who will install it and how long it may take. Ask how you will use essential areas of your home while the work is underway.
When equipment arrives or work is finished, ask who will check it and explain how to use it. Keep instructions, supplier contacts and warranty or servicing details somewhere easy to find. If it feels uncomfortable or does not help with the task you discussed, contact your provider or health professional.
Equipment can change how you manage personal care or household tasks. With your permission, share relevant professional instructions with the people supporting you. Our access guide explains how to arrange an aged care assessment.

Yes. Your aged care assessment identifies the support you need. Some equipment also needs a professional prescription, and an occupational therapist must prescribe home modifications. Your AT-HM provider can explain the next steps for your approved support.
Speak with your AT-HM provider before paying for equipment or booking work. Ask them to confirm the approved item, price and payment arrangements. You may not be reimbursed for something you buy independently.
Ask for a demonstration and clear instructions. Check that you know whom to contact about repairs, servicing or any difficulty using it. Your provider can explain what ongoing support is included.
